In this article, we've listed down all the fields in the Sprout HR Template, what they're for, and how to fill these out.
First make sure you've already set-up the following. Certain fields on the template rely on these pre-requisites:
Now here's how to fill out the template:
All items marked with an asterisk (*) are required.
A. Status after upload - leave this column blank. When you upload the template with incorrect data, an annotated excel template is automatically downloaded - and all the details for correction appear on this column.
B. *Employee ID No - Alphanumeric characters are accepted.
C. *First Name
D. Middle Name
E. *Last Name
G. *Civil Status
H. *Date of Birth - Format is mm/dd/yyyy. Employees added via the HR template will have his or her birthday as the default password.
I. *Contact Number
Here are the accepted formats:
Mobile: 11 or 12 digits (eg. 09171234567 or 639123456789)
Landline: 7 or 9 digits (e.g. 8000000 or 028000000)
Home: 7 or 9 digits (e.g. 8000000 or 028000000)
Work: 7 or 9 digits (e.g. 8000000 or 028000000)
Other: 11 or 12 digits (eg. 09171234567 or 639123456789)
K. *Email - This email will be used to receive email notifications from leave applications.
L. *Department - Select a Department that has already been previously set up.
M. Location - Type in the exact title of the Work Place that has already been set up.
N. Job Title
O. Employee Type - Select if Rank and File, Manager, or Officer.
P. *Employment Status - Select an employment status.
Q. *Hire Date - Format is mm/dd/yyyy.
R. Regularization Date - Format is mm/dd/yyyy.
S. *Biometric ID - Input the biometric ID. Must contain numbers only with a maximum of 10 digits.
T. *Reports To Employee ID No - Input the employee ID of the manager/supervisor (found on column B).

V. *Schedule Type
W.*No of Hours To Work

The following fields repeat from columns X to AR for Monday to Sunday:
X. *Monday is Rest Day? (Yes/No) - Select Yes or No.
Y. *Monday Shift From - Shift To (HH:mm am/pm - HH:mm am/pm) - Follow the format. For example: 08:00 am - 05:00 pm
Z. *Monday Break In - Break Out (HH:mm am/pm - HH:mm am/pm) - Follow the format. For example: 12:00 pm - 01:00 pm
... fill out for Tuesday to Sunday until column AR.
AS. Payroll Pie ID - If your company has a linked Sprout Payroll account, input the employee's Payroll ID here.

AU. *TIN No. - Required.
AV. PhilHealth No
